Our previous relationships with the practices within the PCN instilled confidence in the network for us to provide medicines management support across the PCN.

One of the practices prior to being taken over had a poor rating and one of the areas that required
significant focus on was the medicines management aspect. We worked collaboratively with the lead
pharmacist including 2.5 WTE of our experienced senior pharmacists and developed a plan to
implement and change the policies and procedures around the prescribing

Over a 6 month period we managed to develop the service standardise the processes within the
practice aswell as achieve the numbers below:

This is a huge turnaround for the practice and an achievement for both the practices and the PCN
